The Texas Assisted Living Disclosure Statement

 
     
The State of Texas requires that all licensed assisted living residences post a copy of their completed Disclosure Form and make a copy available to any member of the public who requests one.  
   
   
   
 The purpose of this disclosure form is to provide consumers, prospective residents and their families, with a uniform means of reviewing basic information. It is easier to compare policies and services of several residences if each residence provides the same information in the same way and in the same place on a standard form.

Required information on the disclosure form includes basic fees, additional services offered, any charges for additional services, dementia specific care provided, staffing over a 24-hour period, and possible reasons for discharge.

Of course, the assisted living disclosure form is in no way a replacement for a visit to each assisted living home, and reading the disclosure form is never a substitute for a close reading of the facility contract.
