When an older Texan applies for Medicaid assistance, the Medicaid
caseworker will want to see several documents in addition to the
actual multi-page application form. If you include copies of these
documents with your application form, your application will be
processed faster. Without all the required documents, your
Medicaid caseworker will not be able to process the application.
Be sure to photocopy your documents, and keep the originals. Never
send your original documents with your Medicaid application. You
will not be able to replace them if they are lost.
What Documents to Include with Your Medicaid
Application

1. Copy of Social Security and Medicare cards
2. Copy of Legal Guardianship or Powers of Attorney if you
have one or both (not everyone will have these)
3. Copy of last 4 bank statements (checking account,
savings account, CDs, money market accounts, IRAs) for all
accounts on which the applicant’s name is listed or on which the
applicant is authorized to sign
4. Proof of the source of any deposits which are shown on
account statements
5. Copy of the closing statement for any accounts which
have been either closed during the last 30 months or from which
the applicant’s name has been removed
6. Copy of any trust agreement
7. Copy of an inventory of all safety deposit boxes which
has been verified and signed by a bank representative
8. Copies of all notes, stocks and bonds
9. Copy of the deed(s) for all property transferred within
the last 5 years (houses, lots, land, mineral rights)
10. Copies of all insurance policies (life insurance,
burial policies, medical insurance other than Medicare)
11. Copy of deeds to any cemetery property
12. Copies of ownership documents or leases for oil, gas,
mineral or surface rights
13. Copies of all medical bills you were not able to pay,
especially from the past three months (Medicaid may assist with
paying these)
